How can I Access my Medical Records?
To access your medical records, you will need to make your request in writing by completing the Subject Access Request Form. You can then either hand this in to reception or email to firstname.lastname@example.org.
Please include the words ‘Subject Access Request’ in the subject line of your email.
Our practice has decided to outsource our medical reporting to MediData, which will process your medical records request using their system, eMR.
The main reason for this is to speed up the process time for all requests.
A member of our team will contact you to:
- Obtain your consent to use this service
- Obtain your consent to release your medical records to third party agencies such as Solicitors, Police, Immagration Servies etc
