Social prescribing is an NHS-supported approach that helps people access practical, social and emotional support within their community.
What is a Social Prescriber?
A Social Prescribing Link Worker takes the time to understand what matters most to you and can help you identify and access local services and activities that meet your individual needs. Their role is to support you in improving your health, wellbeing and independence through non-medical interventions and community support.

Social prescribing may be beneficial if you would like support with:
- Loneliness or social isolation
- Emotional wellbeing and low-level mental health concerns
- Housing issues
- Financial worries or benefit advice
- Employment, education or volunteering opportunities

Self-Refer Online
Patients registered with Urban Village Medical Practice, part of the City Centre & Ancoats Primary Care Network, can make a referral directly to a Social Prescribing Service without needing to see a GP first.
