A sick note, or fit note, is a document provided by a healthcare professional that certifies a patient’s inability to work due to illness or injury. It can also offer advice on how a patient might return to work.
When Do You Need a Sick Note?
- Self-Certification: For absences of less than seven days, employees can self-certify their illness. This means you do not need a sick note from your GP for the first week of absence.
- Fit Note Requirement: If you are off work for more than seven consecutive days, including weekends and bank holidays, you will need a fit note from a healthcare professional.
How to Obtain a Sick Note
- Contact the Practice: If you require a fit note, please fill out a sick note request form.
- Hospital Discharge: If you have been discharged from the hospital, the hospital should provide you with a fit note covering the expected duration of your recovery.
- Repeat Fit Notes: For ongoing conditions, you may need a repeat fit note. Please arrange a follow-up appointment with the healthcare professional responsible for your care.
Important Considerations
- Employers may have their own policies regarding sick leave and documentation, so it’s advisable to check with them directly.
- It is inappropriate to request a fit note for absences of less than seven days.
- Fit notes can be issued by doctors, nurses, occupational therapists, pharmacists, and physiotherapists.
