At Urban Village, we understand that there are circumstances where patients may need to grant access to their medical records to a trusted individual, known as a proxy. This can be particularly useful for carers or family members who assist in managing healthcare needs.
As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.
What is Proxy Access?
Proxy access allows a nominated person to access a patient’s medical records on their behalf. This can include booking appointments, ordering repeat prescriptions, and viewing medical history.

- A legal guardian
How to Apply for Proxy Access
- Complete the Application Form: You can download the proxy access application form collect a copy from the reception.
- Provide Identification: Both the patient and the proxy will need to provide identification documents. This ensures the security and confidentiality of the patient’s medical records.
- Consent: The patient must provide explicit consent for proxy access. In cases where the patient is unable to consent, legal documentation may be required.
- Submit the Application: Return the completed form and identification documents to the practice via email at gmicb-mh.uvmp@nhs.net or via reception. Our team will process the request and notify you once access is granted.
Important Considerations
- Proxy access is granted at the discretion of the practice and can be revoked at any time.
- The proxy must agree to maintain the confidentiality of the patient’s medical information.
- Access levels can be tailored to suit individual needs, allowing full or limited access to medical records.

Linked profiles in your NHS account
Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website.
