Sick Notes (Fit Notes) Information

At the practice, we aim to provide clear guidance on obtaining sick notes, also known as fit notes, for our patients. These notes are essential for both employees and employers in managing health-related absences from work.

What is a Sick Note?

A sick note, or fit note, is a document provided by a healthcare professional that certifies a patient’s inability to work due to illness or injury. It can also offer advice on how a patient might return to work.

When Do You Need a Sick Note?

  • Self-Certification: For absences of less than seven days, employees can self-certify their illness. This means you do not need a sick note from your GP for the first week of absence.
  • Fit Note Requirement: If you are off work for more than seven consecutive days, including weekends and bank holidays, you will need a fit note from a healthcare professional.

How to Obtain a Sick Note

  1. Contact the Practice: If you require a fit note, please fill out a sick note request form.
  2. Hospital Discharge: If you have been discharged from the hospital, the hospital should provide you with a fit note covering the expected duration of your recovery.
  3. Repeat Fit Notes: For ongoing conditions, you may need a repeat fit note. Please arrange a follow-up appointment with the healthcare professional responsible for your care.

Important Considerations

  • Employers may have their own policies regarding sick leave and documentation, so it’s advisable to check with them directly.
  • It is inappropriate to request a fit note for absences of less than seven days.
  • Fit notes can be issued by doctors, nurses, occupational therapists, pharmacists, and physiotherapists.
